The Carlington ACT Team, based at the Carlington Community Health Centre, provides mental health treatment, rehabilitation and support services to identified clients with severe and persistent mental illness. Services include: Crisis intervention (clients of ACT only); Education, support and consultation to clients` families; Medication prescription, administration, monitoring, and documentation; Social, interpersonal relationship, and leisure - time skill training; Substance abuse services; Support with activities of daily living; Symptom assessment, management, individual supportive therapy; Work - related services.
